The purpose of SER de Puerto Rico's community programs is to make medical, therapeutic, social and psychological services accessible to impoverished and vulnerable populations, in their communities throughout the entire island. Since 2014, SER has assumed the challenge of reaching the communities furthest away from urban centers to meet the needs of children. Later, after Hurricane María, the community program was expanded to visit people living with disabilities and the elderly on a weekly basis. They are all vulnerable and impoverished sectors, with little or no access to services and who have great difficulty obtaining equipment and materials to meet very basic needs. Through these programs, people with disabilities, the elderly and families receive guidance, support, monitoring of their psychosocial needs and mobility equipment, as well as home assistance equipment and others that medical plans must offer, but they don't get them if doctors don't make the necessary referrals.
The services promote comprehensive health, through screening in the areas of physical, emotional and social functioning, guidance, recommendations, delivery and training in the use of mobility equipment, delivery of hygiene materials, technological assistance equipment for maintenance of daily living skills and links with resources in the communities in order to offer follow-up and support to the participants.
Programs and Services
BE 360 (2017- present)
It is an interdisciplinary care program for people who have lost mobility due to aging or illness. Using a mobile clinic and/or visiting homes, a municipality to be impacted is chosen. According to the identified needs, professionals are brought in a timely manner to provide services and screen communities. Services such as: medical, speech and language therapy, occupational and physical, social work, psychological and others.
SII Initiative – 2019
Care program for older adults (60 years and older) who live independently, in municipalities in the eastern area of the country. It has two (2) components: one educational and open to the community, through which guidance is offered on aspects of mobility, safety at home, proper management of grief, loneliness and sadness, among other topics; The second component consists of visiting homes to explore needs in daily life activities and attending to them through the delivery of Technological Assistance equipment. The goal is to maintain the security, independence and social inclusion of older adults.
Health Expedition (2014- present)
It is an interdisciplinary program based on the early intervention approach. Children from 0-6 years of age are impacted by doctors, physical and occupational therapists, speech-language pathologists, psychologists and social workers who carry out these first screenings on boys and girls; with suspicion of a functional challenge. The goal is to maximize the integral development of the participants, whether or not they have diagnoses. To date, the impacted municipalities are: Aguadilla, Arecibo, Cataño, Coamo, Culebra, Dorado, Fajardo, Orocovis, Patillas, Peñuelas, Loíza, Vega Baja, Yabucoa, and Vieques.
BEING to the North (2021- present)
Its goal is to make psychosocial and support services accessible to people living with disabilities and older adults, increasing harmony in their functioning and in the community, through a biopsychosocial and spiritual approach. It is achieved by visiting homes, identifying needs and bringing services to the home. In addition, interested community leaders are trained to develop support skills, coordination of services in their communities. It was developed in northern towns such as: Florida, Manati, Barceloneta, Vega Baja, among others.
